Authorization & Signature

I hereby authorize: 1) the release of medical information by my primary care physicians, hospitals, nursing facilities and/or outpatient facilities to OT2GO for the purpose of obtaining authorization for services to be rendered or for the payment of insurance claims 2) authorize release of medical information between OT2GO, physicians, insurance company(s), hospitals, nursing facilities, and/or outpatient facilities that may require filing of an insurance claim or appeal a claim on my behalf. I also give authorization to my insurance company to pay OT2GO for services provided. I hereby authorize future contact for care from OT2GO, follow up, and continual treatment, regarding the services that have been provided. My plan of care allows for OT2GO to continue this contact, at any future time, while I receive services from OT2GO.

Cancellation Policy

We understand that schedules change. Please review our cancellation policy to ensure continuity of care for all patients.

Cancellation Policy

We understand that unforeseeable events occur. Kindly provide at least 48 hours' notice if you need to reschedule your visit. This gives us enough time to refill your time slot. If you fail to cancel within 48 hours, your visit will be billed.

Home-based care may be particularly safe during health emergencies. The CDC recommends at-home treatment for older adults and those with chronic conditions, and our clinicians follow protective measures and remain committed to patient safety and community health.

You will receive consistent care from the same clinician, who will contact you by phone to arrange appointments.

We conduct a daily symptom assessment on therapists before the start of each workday. Our therapists will wear protective masks and gloves. Our therapists will use contactless options and social distance when possible.
Occupational Therapists help people across the lifespan to improve health and wellness outcomes, age in place, and live life to the fullest despite injury, illness, functional decline, or disability. We provide outpatient OT services in homes and independent/assisted living communities throughout the Upstate region.
Arthritis, amputations, balance deficits, cancer, COPD, diabetic neuropathy, fractures, general weakness, hypertension, intellectual disabilities, memory deficits, multiple sclerosis, orthopedic procedures, Parkinson's disease, spinal cord injuries, stroke, and traumatic brain injuries.
ADL training, adaptive equipment, assistive devices, cancer rehabilitation, dementia care, dynamic balance, falls prevention, hand therapy, home modifications, low vision orientation, neuromuscular re-education, pain management, productive aging, therapeutic exercise, transfer training, and orthopedic injury treatment.
Eligibility requires physician care, with therapy ordered by a physician, and medically necessary services to treat your condition(s).
Yes, and you should learn as much as you can about the provider to determine which one is best for you and your loved one. Key questions to ask include the provider's experience, Medicare/Medicaid certification, and state licensing status.
