OUR PROCESS BEGINS WITH...
PHYSICIAN REFERRAL
Primary Care Physician (PCP) generates an order for occupational therapy (OT) services based upon recognizable health condition(s).
DATA COLLECTION
OT2GO clinicians review medical records and schedule the evaluation visit.
EVALUATION
The OT performs structured interviews, and clinical observations and administers standardized and qualitative tests.
PROBLEM IDENTIFICATION
The OT interprets assessments, identifies deficits in performance areas, components, and contexts, and makes an OT diagnosis.
DEVELOP PLAN OF CARE (POC)
The patient and OT work together to select goals and intervention strategies based on the OT diagnosis. The POC will be reviewed and signed by the PCP.
COMPLETE REGULAR VISITS
Weekly interventions facilitated by OT2GO team members based on the OT diagnosis.
ASSESSMENT VISITS
OT performs monthly reviews of outcomes to track progress with goals. OT reviews outcomes with PCP.
DISCHARGE
OT performs final review of outcomes to assess maximum rehabilitation potential. OT reviews discharge summary with PCP.
